Good dental care improves your overall health. Our dental plans are designed to help you maintain a healthy smile through regular dental care and fix any problems as soon as they occur.

DHMO Dental Plan

The DHMO dental plan offers in-network coverage only. If you visit an out-of-network provider, you will be responsible for the full cost of services. You must select a primary dentist who will coordinate your dental care needs, including referrals to specialists. You will typically pay a copay for most qualified dental services.

DPPO Dental Plan

The DPPO dental plan offers the freedom and flexibility to choose any licensed dentist you want. You’ll receive the greatest savings when you visit an in-network provider. If you visit an out-of-network provider, you will pay a percentage of the Reasonable and Customary (R&C) fee based on the dentist’s rates. Preventive services are covered 100% in-network and are not subject to the deductible. See plan documents or certificate booklet for additional information.
